Abstrak

The Web-Based Employee Survey Information System at PT Uni Charm Indonesia can be used as a solution in filling out employee surveys at PT Uni Charm Indonesia. The problem faced by PT. Unicharm in carrying out the annual survey is that the HR management division finds it difficult to recap survey data because it is still being done manually by means of HRD distributing the questionnaire to employees in the form of hardcopy or print out surveys. With this information system, it is easier for employees to fill out survey forms and companies in processing reports on the results of the survey. In this study, discusses how to design a web-based employee survey system application at PT Uni Charm Indonesia. The system development methodology used is the Waterfall Model System Development Life Cycle (SDLC), which includes the stages of System Analysis, Design, Coding, and System Testing. The system used uses the PHP programming language, the database server uses phpMyAdmin. The results achieved from this research is a web-based employee survey information system information system. With this employee survey system, HRD can assist in the process of carrying out annual employee surveys and make it easier for employees to fill out surveys so as to minimize paper usage as well as a more efficient data storage system and accurate survey results processing.
